| High Adventure | |
| High Adventure Adult Leader Application Source: Mecklenburg County Council This form is to be completed and turned in to the Scout Service Center by Adult Scouters interested in applying to be a leader for a council contingent to a BSA high adventure base, such as Philmont Scout Ranch, Northern Tier Canoe Bases, Florida Sea Base, or a National Scout Jamboree. Further information about various oppotunities, schedules and application deadlines, and the interview and selection process should be obtained from the council Program Director or from the council camping or high adventure committee. |

| Medical Form | |
| Medical Form, Class 1 & 2 Source: BSA Class 1 Section: Update annually for all participants. Class 2 Section: Required once every 36 months for all participants under 40 years of age. This form is not to be used for adults 40 and over, nor used by High Adventure, Philmont, or Jamboree participants. |

| Medical Form, Class 3 Source: BSA All Class 3 activities require a health examination within the past 12 months by a licensed health-care practitioner. Annually, this form is to be used by adults 40 and over for all activities requiring a phycsical examination and applies to all Wood Badge participants/staff regardless of age. This form is not to be used by Philmont or Jamboree participants (use the forms specific to these activites). |

| Merit Badge Counselor Application Source: BSA This form is to be completed by the applicant and turned in to the Troop for submission to the Scout Service Center. The BSA Adult Registration Application must be attached only if the applicant is not already registered as an Adult Leader. |

| OA Brotherhood Membership Requirements Guide Source: Troop 116 This information sheet will aid the Brotherhood candidate in pointing him to the appropriate references in the OA Handbook and will provide a ready reference as he learns the OA Obligation and Official Song. |

| Tour Permit | |
| Tour Permit, Local Source: BSA For trips and camps under 500 miles. This application form may be completed online and printed. After obtaining required signatures it is to be filed with the local Scout Service Center two weeks in advance of the scheduled activity for proper clearance. |
|
| Tour Permit, National Source: BSA For tours 500 miles or more and tours outside the U.S.A. This application is to be submitted to the local Scout Service Center for approval at least one month before the tour. |
